For funeral services and merchandise. If payment in full is not made in accordance with this agreement, then I (we) will pay interest at the rate of 2% per month on any balance that remains outstanding after due date. If the principle is not paid as outlined, the legal holder of the note may declare the principle due and proceed by law to recover both principle and interest. If this note or interest is not paid as agreed, the under signed jointly and severally agree to pay all costs incurred including such reasonable attorney’s fees as may be allowed by court upon entry of judgement. The undersigned agree that they are jointly and severally liable for the entire indebtedness.

The acceptance of this note does not preclude Edmonton Crematorium and  Funeral Home Ltd. from taking any legal procedure necessary to effect the collection of this Note. The undersigned acknowledges receipt of a copy of this note on the date hereof. *50% Deposit required, balance due in 7 days.

Crematorium, embalming & refrigerated morgue facilities are located at Edmonton Crematorium and Funeral Home Ltd, at 6403 Roper Road Edmonton, AB. T6B3G6

The final location of the embalming and cremation services may be subject to change and, if changed, a list of the then current locations will be provided at the time these services are required. Cremated remains not claimed within five years of the date of cremation will be disposed of in a manner as not to be offensive or to create a nuisance.
